Only approved user fees count: doctor and out-of-hours clinic, physiotherapy, psychologist, blue-prescription medicines, hospital and outpatient clinics, laboratory and X-ray, patient travel, rehabilitation and some dental treatment. Other costs, such as an ordinary dentist visit or medicines without a blue prescription, do not count towards the card.

How it works

In Norway the state pays most of the cost of public health care, but you usually pay a small fee yourself each time. This fee is called a user fee, or in Norwegian egenandel. To stop these fees from adding up for people who are ill often, there is an annual ceiling, the egenandelstak. In 2026 it is NOK 3,278 (about EUR 300 or USD 340). When your approved user fees reach the ceiling, you receive an exemption card (frikort) and pay nothing more for services covered by the scheme until the end of the calendar year. This calculator shows how far you are from the ceiling and when you are likely to reach it.

How to use it

Enter the user fees you have paid so far this year, split by type of service, and the month you are in. Then add what you expect to pay per month for the rest of the year. The calculator adds up your fees, shows how much is left, and estimates the month when you reach the ceiling and how much the card then saves you. Your receipts and the overview on Helsenorge are the best source for what you have actually paid.

Worked example: reaching the ceiling in October

Maria has paid NOK 1,800 in user fees by the end of June and expects about NOK 400 a month. She has NOK 1,478 left until the ceiling of NOK 3,278. At that pace she passes the ceiling in October and gets her card. Her expected total for the year would be NOK 4,200, so the card saves her about NOK 922.

What a GP visit costs

These are the main GP fees in 2026. They all count towards the exemption card. Your GP sets out the exact price list, and you pay the same for a video appointment as for a visit in person.

What counts and what does not

Only approved user fees count. The provider must have the right authorisation and charge the fixed rate for the treatment, and private doctors, physiotherapists and psychologists must have an agreement with the municipality or a health authority. Your GP must also have an agreement with Helfo so that the fee can be settled directly. Ordinary dental care, medicines without a blue prescription, medical certificates and the flu vaccine do not count, so they never bring you closer to the card.

Children and young people

From 1 August 2026, everyone under 18 is exempt from user fees for services covered by the scheme. Earlier the limit was 16. The change applies only to treatment and medicines from that date, and fees paid earlier are not refunded. If you enter an age below 18, the calculator tells you that you do not need to track your fees.

If you are new to Norway

Your fees are registered against your national identity number or D-number, so make sure your GP and the pharmacy have the right number. Coverage depends on your membership of the National Insurance Scheme, which normally starts when you move to Norway for 12 months or more. Ask for a receipt every time you pay, because the receipt is your proof if a fee is missing from the overview on Helsenorge. What the calculator does not cover

It is a planning tool. It does not know which of your fees have actually been registered with Helfo, whether a particular provider or treatment is approved, or whether you are covered by the Norwegian scheme at all. Insurance excesses, private clinics without a public agreement and costs abroad are not included. Helfo decides who gets the card, and the overview on Helsenorge is the authoritative record.

What is the annual ceiling for health care user fees in Norway?
In 2026 the ceiling (egenandelstak) is NOK 3,278, or about EUR 300 or USD 340. Once you have paid that much in approved user fees (egenandeler) in a calendar year, you get an exemption card (frikort) and pay no more approved user fees for the rest of the year. The counter starts again at zero on 1 January. The amount is set by the Norwegian Parliament each year and was unchanged from 2025.
Do I have to apply for the exemption card?
No. The scheme is automatic. Doctors, physiotherapists, pharmacies and hospitals report your approved fees to Helfo (the Norwegian Health Economics Administration), at least every 14 days, using your 11-digit national identity number or D-number. When you reach the ceiling you get the card within three weeks, and you can see it on Helsenorge (the national health portal) after logging in. If you have paid more than the ceiling because a fee was registered late, the extra amount is refunded automatically.
How much does a visit to the GP cost in Norway?
A normal daytime consultation with a GP (fastlege) costs NOK 179 in 2026 and NOK 301 in the evening. The same fees apply at the municipal out-of-hours clinic (legevakt). Consultations with a GP who is a specialist in general medicine cost NOK 235 in the daytime. Laboratory tests cost NOK 64 and an X-ray NOK 287. At NOK 179 a visit you would need about 19 daytime consultations to reach the ceiling on GP visits alone. Your GP has a price list, and the fees are the same for a video appointment as for an in-person one.
Which costs count towards the exemption card, and which do not?
Approved user fees for the GP and out-of-hours clinic, physiotherapy, psychologists, hospitals and outpatient clinics, laboratories and X-ray, medicines and supplies on blue prescription, patient travel, stays at rehabilitation institutions with a public agreement, and treatment of certain dental diseases count. Dressings and equipment needed for treatment, medical certificates, copies of medical records, the seasonal flu vaccine, private providers without an agreement with the health authorities, and ordinary dental care do not count.
Do children and young people pay user fees?
Until 31 July 2026 children under 16 were exempt. From 1 August 2026 everyone under 18 is exempt from user fees for services covered by the scheme. The change is not retroactive, so fees paid before that date are not refunded. In mental health care and specialised substance abuse treatment, young people are exempt until the end of the calendar year they turn 26. Pregnant women also do not pay a user fee for antenatal check-ups at the GP.
I have just moved to Norway. Does the exemption card apply to me?
The scheme is part of the Norwegian National Insurance Scheme. As a general rule you are a member if you live in Norway, and NAV says membership starts from the date you arrive if you plan to stay at least 12 months. You also need to be registered with a national identity number or D-number, because the fees are registered under that number, and to have a GP whose practice has an agreement with Helfo. The calculator assumes you are covered. Short-term visitors follow other rules, so check with Helfo or Helsenorge if you are unsure.
